Mukhya Mantri Lok Sevak Arogya Yojana 2024 is a medical reimbursement programme of the Assam Government which aims at providing medical repayment to govt. employees including pensioners and their dependents at CGHS rates. Candidates need to apply online through the official website mmlsay.assam.gov.in. In order to complete the mmlsay registration process and submit the application form, candidates need the following documents.
- Mobile number for OTP
- Aadhar card
- Bank details
- Primary – Govt ID other than AADHAAR (PAN Card, Voter ID, Passport, etc.
- Employment ID Proof
